Heart disease can develop quietly for years before noticeable symptoms appear. A person may feel healthy, remain physically active and have no obvious warning signs, while changes are gradually taking place inside the coronary arteries.

This is why doctors sometimes use tests that look for early signs of coronary artery disease before a major cardiovascular event occurs.

One such test is the coronary artery calcium score, commonly called a calcium score.

But what exactly does this number mean? Does a high calcium score mean you have a blocked artery? And if your calcium score is zero, does that mean your heart is completely healthy?

Let’s understand what a calcium score can, and cannot, tell you about your heart risk.

What Is a Coronary Artery Calcium Score?

A coronary artery calcium (CAC) score is a measurement of calcified plaque present in the coronary arteries, the blood vessels responsible for supplying oxygen-rich blood to the heart muscle.

Over time, cholesterol, fats, calcium and other substances can accumulate within artery walls and form plaque. This process is known as atherosclerosis.

Some plaque eventually becomes calcified.

A calcium score helps detect and measure this calcified plaque, providing doctors with additional information about the presence and extent of coronary atherosclerosis.

In general, the greater the amount of coronary calcium, the greater the overall burden of coronary artery disease and the higher the long-term risk of cardiovascular events.

How Is a Calcium Score Test Performed?

A calcium score is obtained using a specialised CT scan of the heart.

The scan is quick and non-invasive. It does not involve inserting a catheter into the coronary arteries.

During the test, CT imaging captures detailed pictures of the heart and coronary arteries. Computer software then identifies areas of calcium within the coronary arteries and calculates a score.

The result is commonly expressed using the Agatston score, which takes into account the amount and density of detected calcium.

Unlike coronary CT angiography, a standard calcium score scan generally does not require contrast dye.

What Do Calcium Score Numbers Mean?

Calcium scores are often grouped into broad categories.

Calcium Score of 0

A score of zero means that no detectable calcified coronary plaque was identified on the scan.

For many asymptomatic adults, particularly those without very high-risk conditions, this is associated with a relatively low risk of cardiovascular events over the following years.

However, a calcium score of zero does not guarantee that coronary artery disease is completely absent.

Non-calcified plaque may still exist, and symptoms such as chest pain should not be dismissed simply because the calcium score is zero.

Calcium Score of 1–99

This indicates that some calcified plaque is present.

It suggests evidence of coronary atherosclerosis, although the overall plaque burden is generally considered relatively low.

The significance of the result also depends on factors such as age, sex and other cardiovascular risk factors.

Calcium Score of 100–399

This generally represents a more substantial amount of calcified plaque.

People within this range may have a higher future cardiovascular risk compared with individuals who have little or no detectable coronary calcium.

A doctor may use this information alongside cholesterol levels, blood pressure, diabetes status, smoking history and other factors when deciding how aggressively cardiovascular risk should be managed.

Calcium Score of 400 or Higher

A score above 400 usually indicates a high burden of calcified coronary plaque.

It is associated with a higher risk of future cardiovascular events and warrants careful medical assessment and cardiovascular risk management.

Importantly, the calcium score alone does not reveal exactly how narrowed a particular artery may be.

Does a High Calcium Score Mean You Have a Blocked Artery?

Not necessarily.

This is one of the most important distinctions to understand.

A calcium score measures the amount of calcified plaque, not the percentage of narrowing inside an artery.

Someone may have a high calcium score because plaque is distributed throughout several coronary arteries without necessarily having one severe blockage.

Conversely, a person may occasionally have significant coronary disease that is not fully represented by the calcium score, particularly when plaque has not yet become heavily calcified.

Therefore:

Calcium score = plaque burden and risk information

It is not the same as:

Angiography = evaluation of coronary artery narrowing

These tests answer different clinical questions.

Why Is Coronary Calcium Important?

Coronary calcium is evidence that atherosclerosis has developed within the coronary arteries.

The amount of calcium can therefore provide additional information about a person's future cardiovascular risk.

Traditional heart-risk assessments typically consider factors including:

  • Age
  • Blood pressure
  • Cholesterol levels
  • Diabetes
  • Smoking
  • Family history
  • Physical activity
  • Body weight and metabolic health

But sometimes these factors do not provide a clear enough picture.

For example, someone may have moderately elevated cholesterol but otherwise appear healthy. Their doctor may be uncertain about how aggressively preventive treatment should be approached.

A calcium score can sometimes provide additional information that helps refine this risk assessment.

Who May Benefit From a Calcium Score?

Calcium scoring is generally used for cardiovascular risk assessment in selected people who do not have known coronary artery disease.

It may be particularly useful when a person's estimated cardiovascular risk is uncertain and the result could influence decisions about preventive treatment.

For example, a doctor might consider calcium scoring when deciding whether cholesterol-lowering therapy is appropriate for someone whose overall risk falls into an intermediate or borderline category.

However, calcium scoring is not automatically necessary for everyone.

The decision should consider the person's age, symptoms, medical history and existing cardiovascular risk factors.

Who May Not Need a Calcium Score?

A calcium score may provide limited additional information in some situations.

For example, people who already have diagnosed coronary artery disease or who already clearly require intensive cardiovascular risk management may not gain much additional benefit from knowing their calcium score.

It is also important to understand that calcium scoring is primarily a risk-assessment tool, not an emergency test for someone experiencing acute chest pain.

If someone develops severe or concerning chest pain, emergency evaluation may require tests such as ECG, blood tests and other cardiac investigations depending on the clinical situation.

Waiting for a calcium score would be spectacularly unhelpful when urgent assessment is actually needed.

Can a Calcium Score Predict a Heart Attack?

A calcium score cannot tell you exactly whether or when you will have a heart attack.

Instead, it helps estimate cardiovascular risk.

In general, people with larger amounts of coronary calcium have a higher statistical risk of future coronary events compared with people of a similar age and risk profile who have little or no coronary calcium.

But cardiovascular risk depends on many factors.

A calcium score should therefore be interpreted as one piece of a larger clinical picture rather than as a standalone prediction.

Can You Have a Heart Attack With a Calcium Score of Zero?

Yes, although the risk is generally lower in appropriately selected asymptomatic individuals with a calcium score of zero.

A zero score only means that the CT scan did not detect calcified plaque.

It does not necessarily exclude:

  • Non-calcified plaque
  • Coronary artery spasm
  • Coronary microvascular dysfunction
  • Other cardiovascular conditions

This distinction becomes particularly important when someone has symptoms.

If you experience chest pressure, breathlessness or other concerning symptoms, a previous calcium score of zero should not be used to automatically rule out a heart problem.

Is a Calcium Score the Same as Coronary CT Angiography?

No.

Although both tests use CT technology, they serve different purposes.

A coronary calcium score primarily measures calcified plaque and helps estimate cardiovascular risk.

A coronary CT angiogram (CCTA) provides more detailed images of the coronary arteries and can help evaluate plaque and narrowing within them. It typically requires intravenous contrast.

Your doctor determines which investigation is appropriate based on your symptoms, risk factors and clinical situation.

What Happens If Your Calcium Score Is High?

A high calcium score does not automatically mean you need angioplasty, a stent or bypass surgery.

Instead, the result may prompt a more detailed assessment of your overall cardiovascular health.

Depending on the individual, doctors may review:

  • Cholesterol levels
  • Blood pressure
  • Blood sugar and diabetes status
  • Smoking
  • Diet and physical activity
  • Weight
  • Family history of cardiovascular disease
  • Current medications
  • Presence of chest pain or other symptoms

Preventive strategies may include lifestyle changes and medications when clinically appropriate.

Additional cardiac testing may also be considered if symptoms or other findings suggest that further investigation is necessary.

Can Lifestyle Changes Reduce Your Calcium Score?

This is another area where the number itself can become misleading.

The primary goal of cardiovascular prevention is not necessarily to make the calcium score decrease.

Calcified plaque may remain visible even when treatment successfully reduces cardiovascular risk.

Instead, doctors focus on slowing the progression of atherosclerosis, stabilising plaque and controlling modifiable risk factors such as cholesterol, blood pressure, diabetes and smoking.

Regular physical activity, a heart-healthy diet, maintaining an appropriate weight, adequate sleep and avoiding tobacco can all form part of a broader cardiovascular prevention strategy.

Medication may also be recommended depending on an individual's risk profile.

Should You Repeat a Calcium Score Every Year?

Usually, calcium scoring is not intended to be an annual routine test.

Whether another scan is useful depends on the initial score, age, cardiovascular risk and whether the result would actually change medical management.

Repeated CT scans also involve additional radiation exposure, although the radiation dose from modern calcium scoring is generally relatively low.

Your doctor can determine whether and when repeat testing would provide meaningful clinical information.

When Should Heart Symptoms Be Evaluated Urgently?

A calcium score should never replace urgent medical evaluation when someone develops symptoms that could indicate a heart attack or another serious condition.

Seek immediate medical attention for new, severe or rapidly worsening chest discomfort, especially when accompanied by:

  • Shortness of breath
  • Cold sweating
  • Nausea or vomiting
  • Fainting or severe weakness
  • Pain spreading to the arm, shoulder, back, neck or jaw
  • A heavy, squeezing or crushing sensation in the chest

Even a previous calcium score of zero does not make new or severe symptoms safe to ignore.

The Bigger Picture: A Calcium Score Is One Part of Heart Risk

The coronary calcium score can be a valuable tool for identifying calcified coronary plaque and refining cardiovascular risk assessment.

But the number needs context.

A score of zero can be reassuring in appropriately selected people, but it does not guarantee that every form of coronary disease is absent.

A high score indicates a greater burden of coronary atherosclerosis, but it does not automatically mean that a major artery is severely blocked.

The most useful question is therefore not simply:

"Is my calcium score high or low?"

It is:

"What does this score mean when combined with my age, symptoms, cholesterol, blood pressure, diabetes status, family history and overall cardiovascular risk?"

That is where proper medical interpretation becomes important.
